Prior to making an appointment you must:

1. Familiarize yourself with all information here on the website.
   
2. Have your physician complete and sign the Medical Release Form.
   

All necessary documents must be completed and signed prior to receiving your first treatment. NO ONE will receive a treatment without a Medical Release Form signed by a physician and a signed Waiver Form.
